- Please mark -N/A- such fields that are not applicable to you.

It is essential to indicate your Registration Number. This appears on statements of account sent to you from time to time. In the event you cannot find it and do not remember it, please make sure that you have correctly indicated your National Identity Card (NIC) number. |

Specify the number of the payment instrument. |
|
Place your signature in spaces specified for the purpose. |

| Delivery of Forms |

You can deliver the Form to Arif Habib Investments Limited (formerly Arif Habib Investment Management Limited) in the following ways: |

- Deposit the Form at the nearest distributor.
OR
- Send the Form to Arif Habib Investments Limited (formerly Arif Habib Investment Management Limited).

If you are a resident of Lahore, Multan or Islamabad/Rawalpindi please send your Form along with the payment instrument to our Area Supervisor in your own city. |
|
Residents of Karachi and all other cities of the country should send the Form to our head office in Karachi. |

You can also call us to have your Forms and Payment Instrument picked up from your doorstep. Simply call us at 111-468-378 (111-INVEST) and agree on time for the pick-up.
